We seek Managed Internal Broadband Services on existing equipment for all network switches, wireless APs, firewalls, and applicable servers throughout our three-site district. Van Buren County CSD 1313 1st Street Keosauqua, IA 52565 Ph: (319) 293-3334 Jan. 13, Narrative -We seek e-rate bids for the following … ❖ 5-year licensing for all network equipment below ❖ A walkthrough is required, and the project may be modified after the walkthrough ❖ 85 access points - Ruckus 901-R670-US00 ❖ Cat 6a cable runs to all APs - 85 runs, yellow colored cables required ❖ Cabling rerouted for high school 3rd floor and 4 cables on 2nd floor to the new 3rd floor switch location - 14 cables rerouted ❖ Network Switches: ➢ *Douds Center - 3x Ruckus ICX8200-48ZP2-E, 2x Ruckus ICX8200-24P ➢ *Harmony Center - 3x Ruckus ICX8200-48ZP2-E ➢ *High School - 5x Ruckus ICX8200-48ZP2-E, 2x Ruckus ICX8200-24P ❖ 3x IDF racks at the high school - 3rd floor, shop wing ceiling, and shop wing closet ❖ 3x patch panels - 3rd floor, shop wing ceiling, and shop wing closet ❖ 24x 20ft patch cables for the shop wing ceiling panel to shop wing closet ❖ 2x Vertiv LIEBERT UPS (Mfr # PSI5-1500RT120LINUPS) - 1 for HS 3rd floor rack and 1 for shop wing rack ❖ 12-strand OM4 multimode fiber run from the MDF to HS 3rd floor switch - Approximately 200 ft. ❖ 2x 10G Optical Transceiver Modules for the new fiber run -Installation and testing is required. -Bids must contain all charges including itemized hardware, labor, and other costs. -Bid submissions must be sent to the contact on this form. -No calls or texts. Spam, generic, and/or encyclopedic responses will not be considered legitimate bid inquiries or proposals. 1 -Vendor must be an authorized E-Rate partner of the manufacturer or will agree that no gray market products will be accepted as part of the procurement and must be willing to certify their SPIN via Form 473 SPAC for the related funding year. State and Local Procurement Restrictions -Please include SPIN in the bid. -Inquiries asking the applicant to consider purchasing other services that are not included on the Form 470 will not be considered legitimate inquiries or proposals. -Proposals must separately itemize the cost of E-rate eligible and ineligible services. -Bidder acknowledges their equipment and services are compliant with FCC's recent Order (FCC 19-121) prohibiting the sale, provision, maintenance, modification, or other support of equipment or services provided or manufactured by Huawei, ZTE or any other "covered company" deemed a national security threat. -Any pricing proposed must comply with the FCC Lowest Corresponding Price Rule as required by the Universal Service First Report and Order, and restated in the FCC E-Rate Modernization Report and Order, adopted July 11, 2014. The FCC Lowest Corresponding Price rule prohibits an E-rate services offeror from offering or charging E-rate applicants a price higher than the lowest price the offeror charges to non-residential customers who are similarly situated to a particular school, library, rural health care provider or consortium that purchase directly from the offeror. -Vender is aware and agrees to comply with all applicable Iowa and federal laws and regulations applicable to this filing. Certifications I certify that the applicant includes: I certify that the applicant includes schools under the statutory definitions of elementary and secondary schools found in the No Child Left Behind Act of 2001, 20 U.S.C. §§ 7801 (18) and (38), that do not operate as for-profit businesses, and do not have endowments exceeding $50 million. Other Certifications: I certify that this FCC Form 470 and any applicable RFP will be available for review by potential bidders for at least 28 days before considering all bids received and selecting a service provider. I certify that all bids submitted will be carefully considered and the bid selected will be for the most cost-effective service or equipment offering, with price being the primary factor, and will be the most cost-effective means of meeting educational needs and technology goals. 2 I certify that I will retain required documents for a period of at least 10 years (or whatever retention period is required by the rules in effect at the time of this certification) after the later of the last day of the applicable funding year or the service delivery deadline for the associated funding request. I certify that I will retain all documents necessary to demonstrate compliance with the statute and Commission rules regarding the form for, receipt of, and delivery of services receiving schools and libraries discounts. I acknowledge that I may be audited pursuant to participation in the schools and libraries program. I certify that I have reviewed all applicable FCC, state, and local procurement/competitive bidding requirements and that I have complied with them. I acknowledge that persons willfully making false statements on this form may be punished by fine or forfeiture, under the Communications Act, 47 U.S.C. §§ 502, 503(b), or fine or imprisonment under Title 18 of the United States Code, 18 U.S.C. § 1001. I acknowledge that FCC rules provide that persons who have been convicted of criminal violations or held civilly liable for certain acts arising from their participation in the schools and libraries support mechanism are subject to suspension and debarment from the program.
